Samantha Holcombe, MPH, is a Director, Practice Improvement for the National Council for Behavioral Health. In this capacity she manages the Care Transitions Network, a four-year initiative funded by the Centers for Medicare and Medicaid Services that is working with providers across New York State to build their capacity to participate in value-based payment arrangements and reduce all cause re-hospitalization rates by 50 percent for people with SMI. Ms. Holcombe has extensive experience in designing and implementing complex health programs from the concept/proposal stage through successful implementation and close-out for a variety of donors. Ms. Holcombe holds a Master’s in Public Health from Johns Hopkins University Bloomberg School of Public Health and a Bachelor of Arts in Psychology from the University of Maryland, College Park.
