Missouri Coalition for Community Behavioral Healthcare
Rachelle Glavin joined the Missouri Coalition for Community Behavioral Healthcare in April 2008 and serves as Vice President of Strategic Clinical Initiatives. She began her work at the Coalition as the Disease Management Coordinator for the DMH Net Disease Management Initiative, one of the first integrated care initiatives between the state Medicaid authority, Dept. of Mental Health and MO Coalition, which evolved into the Healthcare Home initiative in 2012. She also assisted in development and implementation of the metabolic syndrome screening policy and Disease Management 3700 Project, which received the 2012 Governor's Pinnacle Award for Quality and Productivity. Rachelle continues to support the development of Healthcare Homes and integrated care in the community behavioral health setting in several areas, including technical assistance, program policy, training initiatives, evidence-based practices, health information technology, population health management and clinical measures and outcomes.
