Jennifer Polello, MHPA, PCMH-CCE is the Director of Clinical Data Integration and Social Determinants of Health for the Community Health Plan of Washington (CHPW). CHPW's network includes 20 Community Health Centers (CHCs) that operate more than 130 clinics across the state, more than 2,500 primary care providers and 14,000 specialists, and more than 100 hospitals. Jennifer is a health care professional with expertise in chronic disease management and clinical care coordination programs and services. Her professional experience includes public health policy and administration, finance, quality improvement, clinical decision support, health information technology and clinical transformation.
Prior to joining CHPW, Jennifer served as the Director of Implementations for leading population health management company Arcadia.io. In that capacity, she worked with health plan and healthcare provider organizations across the country to help them enable their transition from fee-for-service to value-based care.
She has also previously been an Adjunct Clinical Assistant Professor at Washington State University, Spokane and directed the CHER Diabetes Education Center for Inland Northwest Health Services.
Jennifer earned her BASc from Southern Oregon University and her MHPA from Washington State University. Her research has been published in Clinical Diabetes, Population Health Management, the Journal of General Internal Medicine, and the Washington State Journal of Public Health Practice. ADIDAS
