Utilizing Predictive Analytics to Increase the Value of Care

With over a million attributed value-based patients, Advocate Aurora Health has had to continue to innovate in ensuring the right care is targeted to the right needs of patients. Using a cross-continuum, big data platform (that includes claims and clinical EMR data) and predictive analytics, this session will outline successful efforts in targeting outpatient care management resources and interventions to patients at highest risk for unnecessary hospitalization. In addition to more appropriate patient identification, these efforts support a redefined approach to episodic care management: one that focuses on patient education and graduating patients to an enhanced ability to self-manage the condition. This partnership of technology, data science and patient engagement has jointly yielded significant reduction in readmissions and improved the health of a population.
