Using Analytics to Drive Patient-Centered Post-Acute Care

Physician-owned, Summit Medical Group (SMG), is a multispecialty medical group in New Jersey, with 800+ practitioners covering more than 80 specialties and services. In 2017, a growing number of their patients were being served in post-acute settings, including skilled nursing facilities (SNFs), long term care hospitals, inpatient rehabilitation facilities, hospice, and in-home care via home health agencies. However, SMG had limited visibility into the cost and quality of the care delivered, especially at the 20+ SNFs used by its patients. SMG physician leaders will explain how they used analytics to build a preferred network of 4 SNFs and shape a new clinical approach to post-acute care, including SNF integration into care management workflows. By 2019, SMG improved their inpatient-to-home rate, reduced SNF days/K, and reduced readmission rates – improving care while reducing NextGen ACO costs. And at the preferred SNFs, average length of stay was reduced by as much as 43.2%.
