MaineHealth Accountable Care Organization (MHACO) is a network of 10 hospitals, 378 practice locations, and 1,600+ healthcare providers dedicated to measurably increasing the quality and decreasing the cost of care for 380,000+ patients in Maine and northern New Hampshire. MHACO’s coverage area has one of the highest Emergency Department (ED) utilization rates in the country, so they launched the initiative REDUCE (Reducing ED Utilization: A Collaborative Endeavor). MHACO’s educational tools assist patients with real-time decision-making, guiding them to the right care at the right place. Predictive analytics and an enterprise clinical data warehouse pro-actively identify at-risk patients, and help understand the root causes of their utilization. New clinical care work flows help MHACO collaborate with ACO care teams on personalized interventions. Our work as an integrated health system provides our patients the right care, at the right time, at the right place.
