Understanding social determinants of health is critical in achieving health equity. As healthcare organizations use digital technologies and other personalized approaches to deliver better outcomes for vulnerable populations, they still face many challenges.

In this session, our panel of will discuss the state of social determinants of health and explore how organizations are engaging providers and patients to develop effective strategies to promote health and wellness in our most vulnerable and at-risk populations.
