Growing awareness of how SDoH shape health has contributed to efforts to address actionable socioeconomic risk factors through the health care system. These efforts have sparked many innovations. However, the integration of these innovations into existing EHR supported clinical workflows is challenging. Not only is SDoH data commonly captured in unstructured and non-standardized formats, but there are also gaps in and overlap of existing terminologies and codes available to represent SDoH-related activities. A consensus-based approach is warranted to standardize and harmonize SDoH concepts for use in EHRs. In this session we will: present a value proposition for standardizing SDoH data to support documentation across clinical screening, diagnosis, care planning, and treatment activities within an EHR system; discuss how SDoH standardization in clinical settings is critical for interoperable electronic exchange and aggregation across clinical, community-based, research, and population health systems; and share how industry stakeholders can participate in the SDoH Coding Collaborative Project and further advance the validation and testing of SDoH data capture in clinical settings.
