SMART on FHIR Integration to Improve Medication Adherence

Healthcare providers continue to struggle with providing quality patient education to patients that speak other languages. Patient's not understanding their medication regime leads to poor patient outcomes, hospital readmissions and dissatisfaction. This session will demonstrate how these issues were addressed at a large multi-facility hospital system with the use of a SMART on FHIR application in less than one year.
