Risk-Based ACO Success With Clinical Care Communication

Description

Avera Health is a full-spectrum regional healthcare family providing services to a population of nearly 1 million people. Avera participates in several Medicare Shared Savings Program Accountable Care Organizations (ACO), one of which is Prairie View Care Organization ACO. Historically, Medicare patients had to complete a medically necessary, consecutive three-day hospital stay before Medicare would pay for their care in a skilled nursing facility (SNF). The SNF 3-Day Rule Waiver allows eligible SNFs that have partnered with an ACO to bill Medicare for certain patients’ care, even if they haven’t had a prior consecutive three-day inpatient hospital stay before being admitted. With the success of this program hinging on clinical and financial outcomes for the entire span of the patient’s care, risk-based ACOs face data collection and aggregation challenges to prevent redundant and unnecessary care. This presentation covers how Avera tackled these clinical communication challenges.
