Five Best Practices for Improving Transitions in Care with HIE

Providers and payers have a significant opportunity to improve transitions in care with HIE data, but the journey is fraught with challenges. AmeriHealth Caritas is in its fifth year of using HIE data with experience connecting to eight state/regional HIEs. In 2017, the health plan received over 3,000,000 HIE transactions. However, there were a host of challenges that AmeriHealth Caritas had to overcome before HIE data could deliver real value to improve care transitions and other healthcare processes. In this session, this national health plan will describe five best practices that led to its success and highlight the key lessons learned along the way. Among these are obtaining and using HIE data in near real-time, using predictive analytics and other clinical markers to hone in on critical cases, integrating the information with back-end processes, enriching the data to see the whole person needs and closing the loop through bidirectional exchange.
