What happens when major institutions representing seemingly different segments of the community come together to focus on a common goal is quite remarkable. For Neighborhood House Association (NHA) of San Diego and Sharp Medical, the common goal is promoting health and wellness. In this session, Rudolph A. Johnson, III, President and CEO of NHA of San Diego, and Julie Cowan Novak, DNSc, RN, CPNP, FAANP, FAAN, Executive Director and Program Manager of Nemeth NICU Followup Clinic at Sharp Mary Birch Hospital for Women and Newborns will share their experience in leveraging technology to identify and address the needs of underserved and at risk populations. Together, we will explore how the intersection of common data points revealed a new digitally enabled approach to serving the needs of a community in need.
