Improving Preventative Care in Pediatrics through Health and Technology: A Davies Story

Description

The session will review how a QI project to improve pediatric well visit rates in an independent ambulatory pediatric practice led to practice success and improved care. The speaker will discuss challenges that had to be addressed as well as the importance of asthma education for pediatric patients. The speaker will also discuss leveraging the electronic medical record to result in an Asthma Home Management Plan of Care for each family as part of the clinician natural workflow and discharge process and significant reductions of asthma readmission rates.
