Realizing Value and EHR Quality Assurance With Patient-Generated Health Data

10:00am - 11:00am Thursday, March 12
Orlando - Orange County Convention Center
W311A

Description

Roswell Park Comprehensive Cancer Center sought to employ a strategy that would accelerate our commitment to patient engagement and ownership of their personal health record - with the ultimate goal being to empower our patients as an active contributor and editor of their legal medical record. This session will demonstrate how Roswell Park leveraged a multi-application integrated patient generated health data (PGHD) solution to increase efficiency and quality across the continuum of workflows involving the capture, review, validation, and commitment of PGHD into the EHR and other ancillary systems. The value of this implementation is realized through higher referral rates to our ancillary services, increased revenue stream for some consultative services, cost savings from decreased paper utilization, increased patient portal registration facilitating greater access to OpenNotes and other patient engagement services, and more frequent consent for participation in our regional health information exchange.

Learning Objectives

  • Formulate guidelines and steadfast rules governing the deployment of patient-facing questions that prompt patient generated health data
  • Identify the technologies used by each role throughout the workflow from generating the survey of patient-facing questions through reconciling and committing the answers to the EHR
  • Design logic and workflows that facilitate the preparation of patient-specific surveys based on patient-based, condition-based and visit-based attributes
  • List the metrics or combination of metrics that will define a successful implementation of integrated patient generated health data technologies
