Description

Hawai‘i Pacific Health will describe its use of cross-functional teams comprised of physicians, their staff, and IT to implement team-based care and electronic visits. Such initiatives positively transformed the organization's care delivery as it moved toward a capitated payment model. Specifically, Hawaii Pacific Health improved access and the quality of care provided to an evolving patient population which led to improved patient outcomes, a reduction in physician burnout and a positive effect on medical cost trend.
