The speakers will share a case study on engaging with bundled payment arrangements using a patented precise disease classification system. This session will highlight how Hackensack Meridian Health (HMH), with the aid of a technology partner, identified 450 distinct patient types for bundled payments from a 3-year retrospective review of breast cancer patients. Through the technology partner’s new disease classification schema alongside data analytics and efficient EHR data extraction to standardize care, HMH is reducing adverse variance and gaining an understanding of their case mix, one of the primary barriers to entering bundled payment models. The speakers will elaborate on the barriers and share results from HMH’s ongoing commercial cancer bundle program that uses the precise disease classification to maximize clinical outcomes and reduce the total cost of care, along with how to create a data-driven culture and best practices for addressing adverse care variance with physicians.
