Population Health and Data Foster Success in 23 MIPS-ACOs!

This session will highlight how hospital and clinic participants in 23 MSSP Program Track 1 ACOs increased net patient revenue 17% and saw an improvement in 14 of 17 quality measures. The session will highlight how these successful ACO participants used data analytics and population health software to achieve these fantastic results and crush MIPS with top scores. The speakers will review key data analysis concepts including: Enterprise Data Warehouse analytics, care coordination, and ACO Quality Reporting under the Group Practice Reporting Option. Analysis tools will be reviewed including: market-share reports, HCC analytics, ED visit reports, high cost and utilization reports, and risk stratification reports. Care management strategies to be used with information from data analytics will be described and include: chronic care management services, patient cohort strategies, care plan strategies, and reviews of consolidated patient claims data reports.

  • Assess finance and quality outcome data from hospital and clinic participants in twenty-three Medicare Shared Saving Program Track1 ACOs (MIPS-ACOs) that increased net revenue by seventeen percent and demonstrated improvement in eighty-two percent of quality measures
  • Recognize that MIPS scoring and reporting for Track 1 ACO participants can result in 23% bonus payments on Medicare Part B payments in 2019, and 25% in 2020 with NO risk, using the Group Practice Reporting Option (GPRO) that is significantly less demanding than reporting as a single entity
  • Identify key strategies including Enterprise Data Warehouse analytics, care coordination, ACO Quality Reporting under the Group Practice Reporting Option. Analysis tools will be reviewed including these reports: market-share, HCC analytics, ED use, high cost- utilization, and risk stratification
  • Summarize care management strategies to be used with information from data analytics will be described and include: chronic care management services, patient cohort strategies, care plan strategies, and reviews of consolidated patient claims data reports
  • Discuss steps to take now to use data analytics and population health software and strategies to achieve financial success and improve the quality of patient care through successful Track 1 ACO participation
