Payer Enablement: Population Health Management Through HIE

Description

Greater Houston Healthconnect (GHH) has successfully connected more than 24 counties in TX and LA, covering more than 15 million patients and 95% of hospitals in that market. This session focuses on the successful collaboration of a Blue Cross and Blue Shield Plan with GHH and their partner provider delivery systems to enhance value-based care and community health programs. It’ll highlight how they did this by utilizing core GHH services, e.g. patient-matching utility, real-time alert system for member admit/discharge/transfers across state or county lines, and exchanging robust EHR data (C-CDA) to improve the populations they serve. The next phase of work, using electronic health information to mitigate social conditions by building capacity and better connecting members and communities through a clinical/social determinants of health information (SDoH) exchange. BCBSTX awarded grants throughout TX to support the SDoH information exchange, with GHH as one of the recipients.
