Patient Engagement as a Key to Bundled Payment Model Success

To achieve success with bundled payments, providers must empower patients to become active participants in their care and engage patients before admission and after discharge to guide, educate and monitor their recovery. Participating in CMS’ comprehensive care for joint replacement (CJR) mandatory payment model, Froedtert and the Medical College of Wisconsin needed a way to improve communication and support to patients outside the care setting while achieving the minimum patient reported outcomes measures (PROMs) collection rate needed to maximize reimbursement. With a mission in mind, Froedtert leveraged technology to engage with their patients and deliver the right educational content and guidance at the right time.
