Documentation-based exchange via Consolidated Clinical Document Architecture (C-CDA), when implemented correctly, has the power to capture and share a more comprehensive electronic record that improves care. This presentation, led by HIMSS Health Story Project volunteers, describes the purpose of the individual document types specified in C-CDA and explains how C-CDA documents can be organized, rendered, and exchanged to convey a more meaningful and usable patient story. Attendees will learn how to increase the value of these documents for clinicians, payers, and patients by continuously and routinely gathering stakeholder input. This presentation will also discuss ways to increase patient engagement by sharing C-CDA documents with patients and incorporating patient comments.
