Critical Connections: Finding and Linking Medicaid Patients to Healthcare

Description

To curb Medicaid costs that consumed 40% of its budget, the state of Massachusetts created an accountable care organization (ACO) program to help beneficiaries receive needed preventive care and chronic condition management to reduce unnecessary emergency department and hospital utilization. For complex patients with behavioral and mental health needs, ACOs in the Boston and Southeastern Massachusetts region receive support from Aspire Health Alliance, which uses technology identify and intervene with beneficiaries when they arrive in the ED. As a result, patients receive a consultation with a designated care manager and a care plan, which includes medical, mental, and behavioral health services before they leave the hospital. Since adoption, the number of patients enrolling in a behavioral health program has increased by 150%.
