The session will highlight how Hartford HealthCare has streamlined the care across its network of more than 18,000 employees. Hartford HealthCare offers the full continuum of care with six acute-care hospitals, the state’s longest-running air-ambulance service, behavioral health and rehabilitation services, a large physician group and clinical integration organization, skilled nursing and home health services, and a comprehensive range of services for seniors, including senior-living facilities. The session will focus on Hartford HealthCare’s strategies that included integration of 36 different EHR systems spread across hundreds of ambulatory practices. Further, it will focus on how the data coming from sources beyond EHRs (such as ADT), feeds, labs, claims, and many more add up to the difficulties and how Hartford translated this into network-wide opportunities.
