Leveraging Data Analytics to Complement Value-Based Approach

The session will outline how an integrated health network developed growth strategies with the use of smart analytics to succeed in the value-based ecosystem. Mercy Health Network (MHN) is an integrated system of member hospitals and other health and patient care facilities united into one operating organization. With 400+ service locations and 3,500+ providers, MHN currently manages more than 310,000 patients under 20+ value-based agreements. The session will explain how Mercy Network developed a complete picture of patients spread across six regional chapters and delivered enhanced clinical and financial outcomes. This was done through advanced analytics, event-based triggers, and community resources, which ultimately resulted in a 31% increase in annual wellness visits, a 14% increase in primary care visits, a 6% decrease in ED visits and a 7% reduction in 30-day readmission. Overall, there was ~280% increase in cumulative total return for the network.
