Interoperability in Practice: Pharmacist eCare Plan

Description

This interoperability briefing will showcase the implementation of the HL7/NCPDP Pharmacist eCare Plan which is adopted by Community Pharmacists Enhanced Service Network USA (a community-based pharmacy network focused on improving medication optimization activities and patient care services). In this model, pharmacists use the Pharmacist eCare Plan to share medication-related health concerns, interventions and the patient's health outcomes and goals over a period of time. Creating and sharing electronic care plans is essential to integrate care team members and assist payers with case management.

  • Describe the value-based and alternative payment models for pharmacist
  • Describe the importance of electronically capturing clinical services such as electronic care plans for chronic care management
  • Discuss the NCPDP/HL7 Pharmacist eCare Plan using HL7 eCare Plan standards
