Lehigh Valley Health Networks (LVHN) leaders are using the analytics expertise gained over three decades to implement value-based care on a grand scale. By applying analytics, EHR integration, and physician incentives in a coordinated and collaborative strategy, LVHN has helped managed 157,000 lives under care management. The provider has seen a solid return on its investment. For example, six clinical initiatives were identified that were projected to save the healthcare organization $3.1 million. The initiatives ended up saving more than $6 million in a one-year period. Using analytics, care managers and physicians can become aware of population needs and act on those needs in a targeted way. This session discusses foundations for success in value-based contracts and how population analytics data was introduced into the EHR environment to engage physicians in quality improvement.
