Improve Patient Health Through Real-Time ADT Integration

Coordinating the care of the seriously and chronically ill is exacerbated when care teams struggle to find out where their patients are. South Country Health Alliance (SCHA) in Minnesota’s transition of care vision was to be able to proactively plan patient needs post-discharge to facilitate a positive care transition. Historically, this had been challenging because SCHA did not receive notifications for 15-20% of hospital admissions. SCHA set a goal to receive 100% of hospital admission notifications to assist with discharge planning to prevent readmissions. To achieve this goal, the SCHA team knew it needed access to up-to-date information about patient admissions, discharges and transfers (ADT). This session demonstrates how to use innovative technology to transform the care management practice of transition of care management and provides an overview of the real-time HL7 ADT messaging program put in place at SCHA.
