Advocate Aurora Health embarked on a journey to design an outpatient clinic model that was repeatable throughout its healthcare system. The goal? To decrease the patient’s overall length of stay, improve access and efficiency, increase care team communication, and change the way care is delivered in primary care. The result is a collaborative care model for its clinics, where patients receive services within the same exam room rather than moving from location to location, ensuring a more patient-centric experience. Learn how technology is leveraged to support the collaborative care design, while incorporating a self-rooming component that affords patients increased privacy without extending wait times.
