This poster will be displayed in conjunction with the HIMSS20 Career Expo. Posters will be available for viewing through the entirety of the event. Poster presenters will be available for questions from 11:00 am -12:00 pm.

Organizing, understanding and using patient's data to its fullest potential would not have been possible without leveraging tools of data visualization for healthcare providers. However, due to lack of availability of data sources like death data, social and behavioral data can lead to difficulty in predicting patient's health outcome. Understanding the journey of a chronic disease patient by comparing with similar patients as a benchmark to generate actionable insights is the utopia for every healthcare provider. This poster briefly highlights effective communication of patient's health outcome predictions by leveraging behavioral and social determinants of health. The prevalence of chronic conditions is high and increasing. However, healthcare is struggling to get upstream of the problem, to prevention.

  • Define the importance of identification of relative risk to improve health outcomes
  • Analyze graphical visualization of a patient's journey versus journey of similar patient group to highlight gaps in treatment and variation in behavior leading to actionable insights using real world evidence
