As Providence St. Joseph Health forges an intentional and integrated five-year plan to improve community health status over its seven-state service area, the road is guided by Community Pathways to Health (CPH), our internally-developed data and analytics platform that combines clinical, demographic and patient-reported information with block-level measurements of the social determinants of health. This one-of-a-kind tool is helping PSJH better identify areas of need where patients are most vulnerable, develop a more proactive community benefit plan, improve care management, become proactive with social determinants of health, further refine mental health and well-being, and improve services for the Medicaid population. Breakthrough tools like CPH make us a proactive community partner that responds before neighbors become patients.
