Facilitating Improved End-of-Life Care

Description

There is great potential for the EHR to facilitate exceptional end of life care and support advance care planning. Attendees will learn how one large health system designed and implemented an Advance Care Planning Module. Although initially believed to be a simple project, our path to implementation turned into a prolonged but ultimately highly successful journey that was a result of the efforts of a dedicated, multidisciplinary team working towards a common goal. This comprehensive source of information related to end of life care included sections on surrogate decision makers, links to advanced directives and other advance care planning documents, information about code status and documentation tools that guided goals of care discussions. Discussion will also include how this tool can be successfully operationalized and used to support adoption of a DNR policy.
