Expanding Access to Advance Care Plans with HIE

Description

Advance Care Planning (ACP) has significantly improved multiple health outcomes, particularly for patients with serious illness; including increased compliance with patient wishes, reduced hospitalization at end-of-life, improved quality of life, and lower costs of care. While the number of personal Advance Directives has grown significantly in recent years, the majority of people in the U.S. still have not engaged in the process. Making Choices Michigan (MCM) is an independent non-profit dedicated to addressing this challenge by facilitating advance healthcare planning in West Michigan. MCM partnered with Great Lakes Health Connect (GLHC), the leading health information exchange in Michigan, to upload Advance Directives into an individual’s Virtual Integrated Patient Record (VIPR). This session will identify the opportunities and challenges in launching a community-based approach to Advance Care Planning, and the value of HIE for storage and retrieval of AD information.

  • Examine the challenges and barriers involved in gaining broad adoption of personal Advance Care Plans
  • Identify the four main components of a successful, freestanding, community-based ACP program
  • Demonstrate the value and advantages for patients and providers of leveraging health information exchange for the electronic storage and retrieval of Advance Directive documents
