HEALTH care is the sum of SOCIAL care and MEDICAL care.  We have historically focused too much on Medical care, and too little on Social care, which has resulted in .. too much spending on preventable medical issues.  In 2019, the importance of the Social Determinants of Health (SDOH) in helping individuals stay healthy is common knowledge, but not yet common practice. What is the disconnect?
 
This session is an invite-only roundtable for thought leaders determined to prove the outcomes and ROI of treating social needs. We will identify the disconnects in the current system, and the opportunities for innovation  to play an important role in the solution: technology, business model, patient engagement, payer-provider collaboration, and physician-care team-community collaboration. Social Care + Medical Care = Health Care.
