Data-Driven Change to Meet High-Stakes Performance Goals

4:15pm - 5:15pm Tuesday, March 10
Orlando - Orange County Convention Center
W308A

Description

As a participant in New York State’s DSRIP program, OneCity Health must reduce avoidable hospital use by 25 percent within five years for its 750,000 attributed lives, and improve performance in 56 metrics. After failing to meet three major milestone goals in Measurement Year 3 (MY3), the organization faced a six-month window to achieve DSRIP MY4 metrics. With millions of dollars in financial incentives at stake, the OneCity team launched an ambitious effort to procure a data platform capable of taking an analytic deep dive into the issues for quick and targeted quality improvement interventions. Credible data, benchmarking tools, and constant performance feedback helped achieve buy-in from stakeholders. Applying a data-driven approach, OneCity Health quickly uncovered opportunities to reduce avoidable admissions and readmissions for asthma patient populations. Analyze and assess the challenges, triumphs, setbacks and lessons learned in OneCity Health’s race to meet its performance goals.

Learning Objectives

  • Demonstrate how to use data analytics to drive change and achieve quality improvement goals in a high-stakes environment and within a limited timeframe for attaining financial incentives
  • Identify the quickest path to obtaining and analyzing the data required to inform intervention strategies when internal data sources, analytic skillsets, and public data are not available
  • Design and implement methods to address competing priorities and achieve buy-in from clinicians, administrators, and community partners for proposed intervention strategies
  • Apply short-term fixes that capitalize on immediate improvement opportunities while simultaneously working on long-term, sustainable solutions that can be implemented system-wide
