In this session, attendees will learn how Northwell Health, one of the nation’s earliest adopters of population health, is using informatics and data to develop a comprehensive strategy to identify at-risk individuals and make connections to appropriate community resources to reduce unnecessary hospital and ED utilization and improve population health outcomes. As Northwell will explain, this requires leveraging data analytics, artificial intelligence, interoperability, and community-based organization engagement.
