This session will focus on data across the continuum of care and the importance of data to population health management.  You'll hear from a speaker on how to identify source data and the importance of it's integration to transitions of care.
This session is part of a special program called Nursing Informatics Symposium: Beyond the EHR: Changing Care Models, Changing Roles. Extra fees and separate registration is required.

  • Outline challenges with data flow across the continuum
  • Define how data can be used to change individual behavior
  • Discuss how nurse informaticists can ensure proper systems translation and communication
