This session will outline how we leveraged technology to developed a preferred provider network with our skilled nursing facility partners. The session will outline challenges with gathering information, technology used to capture discrete metrics, how we drive a continuous improvement, measure performance and hold SNFs accountable as a care partner. Focus time will be spent on how we engage out patients with data to make informed decisions related to skilled nursing care and our preferred provider network. Finally, we will discuss the value this program has brought to our organization, patients and care providers.
