Countdown to Discharge: An Effective Tool in Facilitating Timely Discharges

Our organization has provided the interdisciplinary team with a countdown tool for inpatient clinical headers, to communicate to all, an anticipated discharge date which would influence the patient’s care and stay. Although the tool is visible to all, the drivers of this tool are the case managers and providers. These two roles are able to trigger the countdown by entering/updating an Expected Discharge Date (EDD) via the Expected DC Date navigator. The implementation of this tool is meant to foster a situational awareness of the patient’s EDD for all clinicians. Communication of the EDD in the patient’s header allows the interdisciplinary team to tailor their workflow in consideration of the patient’s discharge date. Timely discharges allow the organization to eliminate unnecessary cost from extended patient stays, maximize revenue for bundled patients, and appropriately allocate resources based on need.
