Description

The referral management project was initiated due to a need for an improved process for referral tracking. It examined the workflow beginning with referral order placement and concluding with closing the loop. Custom tools were created to improve the efficiency and effectiveness of the patient referral process, standardization of the referral workflow process, insure patients receive necessary services on a timely basis, consulting providers receive pertinent patient information, and to encourage and facilitate patient engagement as a partner in the process.

  • Identify what triggered the initiation of the referral management project
  • Describe three challenges faced by referral coordinators with the previous workflow (prior to the Closing the Loop project)
  • Assess three ways the new process has remediated the challenges identified with prior current state
  • Define Closing the Loop as it pertains to the referral management workflow
  • Identify the vision for future state reporting
