As the growth and popularity of value-base contracts increases, ACOs, CINs and health systems struggle to engage physicians in key practice initiatives that promise to optimize the health of the patient population while decreasing costs. Providers are reluctant to change care delivery practices, believing the source of network cost and variation in best practices lies beyond their patient population. This educational session describes best practices and lessons learned by Alliance Cancer Specialists and Shore Quality Partners in their multi-year population health management journey. The speakers share best practices and lessons learned across their physician-led organizations to create a new culture and network initiatives embraced by both providers and practice stakeholders. Once physicians comprehend data’s value in transforming patient care, they feel empowered and become strong advocates; they are more motivated to provide customized care, reduce variability and improve outcomes.
