Description

This session will address how to leverage digital health innovations to improve chronic disease adherence to care plans, improve population health, and enhance patient satisfaction. Ochsner Health demonstrate how to implement a governance model that enables an innovation program for a large health system. Finally, Ochsner will highlight a series of digital medicine innovations and workflows which have improved patient outcomes and the overhaul population health of the Ochsner community – Digital Hypertension – Connected Mom – Digital Diabetes.
