In this session, the Big Red Consulting Team from Cornell University’s Sloan Program in Health Administration analyzes and provides sustainable models of growth for the population management tactics employed by Centura Health to reduce patient readmission rates by focusing on workflow, information technology, and community and hospital advocacy in both their rural and metropolitan facilities.

  • Recognize benefits of Lean Six Sigma to improve patient care
  • Create comprehensive discharge intervention to reduce patient readmissions and increasing costs
  • Compare Comprehensive Primary Care Initiatives by CMS and recognize the benefits of upgrading EHR to meet new CPC requirements
