Designed to help organizations and health systems evaluating capitated contracting or transitioning to higher risk levels, this session describes the strategy used by Montefiore Health System to implement value-based models, understanding that patients’ health extends well beyond healthcare settings. Its model promotes accountability for performance through contractual arrangements, with healthcare partners that prioritize technology implementations to scale community and patient engagement. The session also details Montefiore's use of consumer engagement technology tools that leverage artificial intelligence, clinical claims and social/behavioral data, to engage patients in care and enable care coordination/management. Through real-world examples, presenters will detail strategies for prioritizing patient engagement and care coordination, using a patient-centered approach to population health management.
