Description

Social determinants of health are an underlying cause of today’s major societal health dilemmas including obesity, heart disease, diabetes, and depression.

In today’s session, attendees will learn about developing business case to address social determinants of health. Some of the challenges surrounding data in healthcare. Framework for leveraging data and analytics to adjust for social risk factors. Ways to integrate platforms and technology into provider workflow to address social determinants. Finally measuring the ROI of addressing social determinants.
