Description

With the current emphasis on value-based care, healthcare organizations are pivoting towards capabilities that address the whole patient and not just the condition. Medical care only accounts for between 10% and 20% of health outcomes, whereas Social Determinants of Health (“SDOH”) account for 80% to 90% of outcomes. There is a huge opportunity to impact the healthcare of populations via SDOH – but the challenge: to incorporate SDOH data via interoperability resources into the patient record.
