Learn how both policy and technology advances are enabling the University Hospital Southampton NHS Foundation Trust (UHS) to share its award-winning innovations with other UK National Health Service Trusts. Hear how Global Digital Exemplar, UHS will blueprint their journey and lessons learned to help deliver digital capabilities more quickly and cost-effectively to others. This session will detail how a successful interoperable PHR was implemented to eliminate data silos, allowing information to flow freely between organizations and care teams which helped reduce unnecessary appointments for patients, improve communications and increased patient engagement. Hear how an open platform can work as a solution for sharing across regions to encourage other entities and third-party developers to hook into the platform with the ability to build apps and connect wearables, so they may further their reach into patient-centered care and promoting interoperability across the UK.
