The Population Health Management Forum is a new event that will focus on social determinants of health (SDoH) and share how leading healthcare organizations are using digital technologies and other personalized approaches to transform the patient experience and deliver better outcomes for our most vulnerable populations. Addressing SDoH is critical to achieving health equity and closely aligned with the HIMSS vision to realize the “full health potential of every human, everywhere and to reform health globally through information and technology.”

The forum will help attendees gain a deeper understanding of how leading healthcare organizations engage providers and vulnerable patients to develop effective strategies to promote health and wellness. Expert speakers will share best practices and lessons learned on how to maximize the effectiveness of current technologies in patient engagement – as well as the human role in connecting care.
