Rural communities experience some of the same challenges to good health as do many urban communities. The big difference: Seniors in rural communities often face a "double jeopardy," experiencing the same challenges but without the benefits of current healthcare technology.

 

To address this, HIMSS and Accenture launched the SDOH Tech Challenge, "Seeking Tech Solutions to Improve Healthy Aging in Rural Communities." Teams were encouraged to address three key areas: cognitive care, care coordination, and disease management, and social isolation.

 

In this presentation, Leslie Evans, HIMSS director of professional development will be joined by thought leaders with expertize in policy, innovation and clinical practice to discuss the unique challenges posed to seniors aging in rural communities and to highlight the winning solution that uses the connective power of information and technology to increase access to resources and services in rural communities.
