Two physicians who practice internal and geriatric medicine observed an increasing mismatch between the needs of frail patients and the outcomes of standard of care interventions prescribed to them. They set out to understand how care, provided with good intention, can create a chaotic, inefficient and, ineffective experience when individuals are frail, and ultimately, created a multi-award winning clinical model to systematically address gaps in care. They sought the assistance of consulting services to help advise them on creating technology-enabled tools and processes that were needed to effectively socialize the model to benefit the masses.
