With the recent changes in provincial health model which is focused on delivering patient-centered care, NYGH has been working with our community partners on a way to build on our collective capacity and our experiences to meet the needs of community. Along these efforts, we have taken a data-driven approach towards understanding determinants of health of the community population. Using analytics we established relationships between patients we see at the hospital and compared it to local community demographics. Using GIS to create mapping hotspots, we are looking at disease prevalence distribution including COPD, asthma, diabetes and high blood pressure and to link it with our patients diagnosed with these conditions. In this session, we will share examples of these solutions with live demonstrations of some of our mapping dashboards we built in our BI platform. We will walk through the steps taken for building data model from disparate sources including city and provincial open source datasets and linking it with patient data.
