Indiana University Health is the largest IDN in the state of Indiana. It has a 14-year history with Electronic Health Records (EHR) and, like many IDNs, it expanded its EHR organically as it grew organically. This resulted in an unsustainable environment whose hallmarks were epidemic-level clinician burnout, workaround-driven variability of workflow, an order of magnitude more alerts and nursing tasks than national average, and slow EHR installation. This is a story of enterprise evangelism, governance restructuring, performance improvement, aggressive reversion to vendor standard, and clinico-technical change control – all received with optimism and engagement from end users. Documentation and total EHR time has fallen greatly; performance metrics are now best-in-class; physician burnout has dropped.
